This partnership between DayBreak Adult Care Centers, The Presence Care Project, and the Institute on Aging shared the collaborative mission to support Seniors living with dementia.
Their goals were to:

Institute on Aging
The pilot Senior Care Co-op model was launched with great success at the Institute on Aging’s existing Adult Day Program in San Francisco in June of 2021.
Pilot Outcomes included:

Reduced costs for programs which in turn increased affordability
